How to Make a Discord Application Form (Step by Step)

By the Toscio team · Updated 18 October 2026 · 7 min read

To make an application form on Discord, you have two choices: link an external form (like Google Forms) or use a bot that opens a native Discord form when members click a button. Native forms are usually the better choice for staff, whitelist and team applications, because applicants never leave Discord and you can give roles in one click.

This guide compares the options honestly, then walks you through a full setup, step by step.

Your options at a glance

External form + webhook Ticket bot Bot asking in DMs Native Discord form
Applicant stays in Discord No Yes Yes Yes
Verified Discord identity No (they type their username) Yes Yes Yes
Works with DMs closed Yes Yes No Yes
Answers easy to compare Yes (spreadsheet) No (chat history) Partly Yes
Role given on acceptance Manual Usually manual Depends Usually automatic
Question types Many (choices, files…) Free chat Text Text, choices, checkboxes, files
Setup effort Medium to high Low Low Low

There is no perfect option for every server. Here is what each one is good at.

Option 1: Google Forms (or any external form) + webhook

You create the form on a website, post the link in a channel, and optionally send each response to a Discord channel through a webhook.

Good for: very long questionnaires, questions that branch depending on earlier answers, and teams who like working in spreadsheets.

Watch out for:

  • Applicants leave Discord, and some never come back.
  • Anyone can type any username, so you are never fully sure who applied.
  • Posting answers to Discord needs a small script or an automation tool, which can break.
  • Accepting someone means finding them on the server and giving the role by hand.
  • Replying to applicants is manual too.

Option 2: a ticket bot

The applicant opens a ticket, a private channel is created, and they answer questions in chat with staff.

Good for: small servers where every applicant gets a conversation anyway.

Watch out for:

  • Staff ask the same questions over and over.
  • Answers are spread across chat messages, hard to compare side by side.
  • Lots of open channels to clean up.

Option 3: a bot that asks questions in DMs

The bot sends each question in a direct message and waits for the answer.

Good for: a conversational feel.

Watch out for: many members have DMs closed for the server, and the bot cannot reach them. Applicants also drop off halfway through long DM questionnaires.

Option 4: native Discord forms (modals)

The member clicks a button such as Apply, and a form pops up inside Discord. They type their answers and submit. The bot posts the application in a staff channel with buttons to decide.

Good for: most staff, whitelist and team applications. It is fast for the applicant, the identity is the real Discord account, and decisions can trigger roles and messages automatically.

Limits to know:

  • Discord shows up to 5 fields per form window. Good bots chain several windows for longer forms.
  • Question titles are limited to 45 characters, with an extra description line under each one.
  • Discord forms used to be text only. They now also support single-choice buttons, dropdowns, checkboxes and file uploads, so you no longer need an external form for a screenshot or a rules checkbox. Check that your bot supports them: Toscio does, on every plan.

Which one should you choose?

  • You recruit staff, run a whitelist or recruit players: a native Discord form. It is the least friction for applicants and the least admin work for you.
  • You need a 40-question survey with branching logic: an external form. For choices, checkboxes and files alone, a native form now does the job.
  • You want a free-form chat with every candidate: a ticket bot, or a native form plus an interview thread.

Want a link you can also share outside Discord? With Premium, Toscio can offer the same form as a web form: applicants log in with Discord, so you still know exactly who applied, and accepting still gives the role in one click. It is also how banned members can send a ban appeal.

Step by step: make an application form with Toscio

Here is how to set it up with Toscio, a bot built around native Discord forms. It takes about two minutes.

Step 1: prepare your roles and channels

Before you start, create:

  • a channel where members will see the Apply button (for example #apply);
  • a private channel only your staff can see (for example #applications);
  • the role to give on acceptance (for example @Whitelisted, @Trial Mod or @Member);
  • optionally, a role to remove on acceptance, like @Applicant or @Visitor.

In Server Settings > Roles, drag the bot's role above the role it must give. Discord does not let a bot manage roles placed higher than its own. See permissions for the full list.

Step 2: invite the bot and run /toscio setup

Invite Toscio, then type /toscio setup and fill in:

  • template: RP whitelist, Minecraft whitelist, Staff recruitment or Team / esport recruitment;
  • channel: where members apply;
  • staff-channel: where your team reviews;
  • role: given on acceptance;
  • staff-role (optional): pinged on each new application.

The Apply panel is posted right away.

Step 3: edit the questions

Open the dashboard at app.toscio.com (available in 8 languages, including English) and go to the form editor. For each question you can set:

  • the title (up to 45 characters) and a description (up to 100);
  • the answer type: short text, long text, single choice, multiple choice (checkboxes) or file(s);
  • required or optional;
  • for text: a placeholder and a minimum and maximum length.

You can also edit the panel text, the acceptance message, the decline message, the roles to give or remove, and the cooldown after a refusal. Conditions to apply, like a minimum Discord account age or a required role, keep alt accounts out before they even see the questions.

The free plan includes one open form with up to 5 questions. Premium (€4.99/month or €49.90/year per server) raises that to 25 questions per form, shown five per step, and up to 25 forms, and adds staff votes, CSV export and the web form.

Step 4: test it yourself

Click Apply with an account that is not staff (a friend's, or a test account). Check that:

  • the questions read well on mobile;
  • the application appears in the staff channel;
  • accepting gives the right role and sends the welcome DM.

Step 5: review applications

Each application arrives as a card with the answers, any files attached, the applicant's Discord account age and how long they have been on your server. Your team can:

  • Accept: roles are given and removed, and the applicant gets a DM in their Discord language;
  • Decline with a reason, which is sent to the applicant;
  • Interview: open a private thread with the applicant to ask more.

You can also work through pending applications from the review queue in the dashboard.

Members can only have one pending application at a time, and a cooldown applies after a refusal (24 hours by default, adjustable from 0 to 720 hours). Members can see or delete the data kept about them with /toscio privacy.

Best practices for any application form

Whatever tool you use, these habits get you more applicants, and better ones.

Keep it short

Every extra question loses some applicants, often the busy, competent ones. Five to fifteen questions is plenty. Keep follow-ups for an interview.

Explain why you ask

A line under a question changes the quality of answers. “Your age” gets a number. “Your age, we require 16+ for staff” gets an honest number and fewer awkward surprises later.

Ask only what you need

Do not ask for real names, addresses, phone numbers or social media unless you truly need them. Age, availability and experience are enough for almost every community.

Set expectations on response time

Write it in the panel text: “We answer within 3 days.” It reduces “did you see my application?” messages, and it keeps your team honest.

Decline with a reason

A short, respectful reason (“Backstory too short, try again with more detail on your character's goals”) turns a rejection into a better second application. Silence turns it into a complaint in #general.

Use a cooldown

Without one, some people re-apply every ten minutes with the same answers. A 24-hour to 7-day cooldown is usually enough.

Close the form when you are not recruiting

A form that stays open while nobody reviews it frustrates everyone. Close it in the dashboard and reopen it when your team has time.

Questions

Does Discord have a built-in application form?

Discord's built-in tools (rules screening, onboarding questions and, where available, requests to join) act at the moment someone joins the server. For applications to a role inside your server, like staff, whitelist or team, you need a bot that opens a Discord form, or an external form like Google Forms.

How do I make an application on Discord for free?

The simplest free option is an application bot with a free plan. With Toscio, for example, the free plan includes one open form with 5 questions, staff review, roles given on acceptance and DMs to applicants. Google Forms is also free but needs a webhook to post answers in Discord, and you give roles by hand.

Can I use Google Forms for Discord applications?

Yes. Share the form link in a channel and, if you want answers to appear in Discord, connect it to a webhook with a script or an automation tool. The downsides: applicants leave Discord, you cannot be sure who filled it in, and roles must be given manually.

How many questions should a Discord application have?

Five to fifteen. Five is enough to filter out low-effort applicants; past fifteen, good candidates start giving up. Keep the rest for an interview.

How do I make a bot give a role when an application is accepted?

Use an application bot with an accept button that gives a role, and make sure the bot's own role is placed above that role in Server Settings > Roles. Discord does not let a bot give a role that is higher than its own.

Your next application is two minutes away.

Add Toscio, run /toscio setup, pick a template. That’s it.
