Discord form bot: build a form, review it as a team, give the role
A Discord form bot posts a button on your server. When a member clicks it, a form opens, and their answers arrive in a staff channel with buttons to accept or decline. Accepting can give a role and send a message, without anyone copying answers from a spreadsheet. That's the difference with an external form: the bot knows who answered and handles what happens next.
This page explains the three ways to run forms on Discord, compares them, and shows what you can build with Toscio, our form bot.
What a Discord form bot does
- A panel with a button. You post a message such as "Staff applications are open" with an Apply button in the channel of your choice.
- A form inside Discord. The member clicks and answers in a Discord window: text, choices, checkboxes, files. They never leave your server, and closed DMs don't matter.
- A card for your staff. The answers arrive in a private staff channel, in the same layout every time, with the applicant's account, Accept, Decline and Interview buttons.
- The follow-up, automatically. Accepting gives the roles and messages the applicant; declining sends your reason and can make them wait before applying again.
Three ways to run forms on Discord
When people search for "Discord forms", they mean one of three things.
Discord's own "Apply to Join". Discord can ask questions to people who want to join your server, and an admin approves each request. It's built in and free, but it only covers joining: one questionnaire, no roles given on approval, no staff votes, nothing for staff, team or partnership applications (checked 22 Oct 2026).
An external form linked to Discord. You build the form on Google Forms, Tally, Jotform or similar, post the link, and optionally send each answer to a channel with a webhook or an automation tool. Great for long surveys with branching. For applications, the gaps show quickly: anyone can type any username, so you're never sure who answered, and roles and replies are done by hand. Zapier can give a role, but only if you already have the person's Discord ID.
A form bot. The form lives in Discord, tied to the member's real account, and the bot does the follow-up: staff review, roles, messages. This is the right tool as soon as someone has to decide on the answers.

In Discord or on the web
Inside Discord is the default: the fastest for the member, and the identity is their Discord account. Discord windows hold 5 fields, so Toscio shows longer forms in steps of 5 and saves the answers at each step.
On the web (Premium), the same form is a page with Discord login, with every question on one page. You get a link to share anywhere, and you can open it to people who aren't on your server yet: once accepted, they join in one click with their roles. Banned members can't see your server, so this is also how they send a ban appeal. See web form and ban appeals.
What you can build
- Staff and moderator recruitment: questions, a graded rules quiz, a team vote, a trial period. See staff applications and our 58 staff application questions.
- Member applications: approve people before they get access, or before they join. See member applications.
- Ban appeals: banned members apply from the web; accepting lifts the ban. See ban appeals.
- Partnership requests, creator programs, esports tryouts, beta testers, event sign-ups and whitelists for game servers.
Each form has its own questions, staff channel, roles and language, so a single server can run several at once.
Questions and conditions
Five question types, all on the free plan and all inside the Discord form: short text, long text, single choice (buttons or a dropdown), multiple choice (checkboxes or a multi-select list) and files (1 to 10, images only or any type). A single required checkbox makes a clean "I have read the rules".
You can also set who may apply: minimum Discord account age, minimum time on the server, a required role or an excluded one. Choice questions can become a graded quiz that declines or accepts automatically at the pass mark you set. Everything is in the form editor.
Reviewing as a team
Each application arrives as a card in your staff channel and mentions your staff roles. Reviewers can accept, decline with a preset or custom reason, open a private interview thread, or move the applicant to a trial stage before final acceptance. Notes stay on the card for the team, and the card shows the person's previous applications.
With Premium, staff vote For or Against on the card, and the application is decided on its own when the number of votes you set is reached. Reviewers can also work from the dashboard without needing Manage Server. See reviewing applications.

Free or Premium
The free plan covers most servers: 2 forms, 20 questions per form, unlimited applications, several roles, the graded quiz, the trial stage, notes, history and staff access to the dashboard. Premium, at $4.99/month or $49.90/year per server, adds 25 forms of 25 questions, staff votes, the web form, ban appeals, signed webhooks and CSV export. Your first form comes with 14 days of Premium, no card needed. See pricing.

Questions
Does Discord have built-in forms?
Only for joining a server. Discord's "Apply to Join" asks questions to people who want to join (short text, paragraph, single choice, accepting the rules) and an admin approves each request. It gives no roles, has no staff votes and can't be used for staff, partnership or team applications (checked 22 Oct 2026). For those, you need a form bot.
Is there a free Discord form bot?
Yes. Toscio's free plan includes 2 forms with up to 20 questions each, unlimited applications, a staff review channel, several roles given on acceptance and messages to applicants. Premium ($4.99/month per server) adds 25 forms, staff votes, the web form, ban appeals and CSV export, with a 14-day free trial.
Can members fill in the form without leaving Discord?
Yes. The member clicks a button on your server and the form opens as a Discord window. Longer forms are shown 5 questions at a time, because Discord windows hold 5 fields. With Premium, the same form is also available as a web page with Discord login.
Can people who aren't on my server fill in a form?
Yes, with Toscio's web form (Premium). It can be open to people who aren't members yet: they log in with Discord, apply, and once accepted they join your server in one click with their roles. It's also how banned members send a ban appeal.
How do I connect Google Forms to Discord, and why use a bot instead?
You can post Google Forms answers to a channel with a webhook, through Apps Script or an automation tool like Zapier, IFTTT or Make. It works for surveys. For applications, a bot is simpler: it knows which Discord account answered, and accepting gives the role and sends the message without any extra setup.
Can a form bot give roles automatically?
Yes. With Toscio, you choose up to 10 roles to give and 10 to remove when an application is accepted. The bot's role must be placed above those roles in Server Settings > Roles.
Can I use it for a survey or an event sign-up?
For sign-ups that someone approves, such as an event with limited places, a tryout or a beta test, yes. For an anonymous poll with charts of the results, a survey tool or a poll bot fits better: Toscio is built for answers that a team reviews.
Which languages does Toscio support?
English, French, Spanish, Portuguese, German, Turkish, Polish and Russian: the bot, the forms, the messages to applicants and the dashboard. Each form has its own language.
